Referrals are only accepted from licensed physicians or allied health care providers with a valid Alberta practitioner identification number (PRAC ID)

Please include any relevant information, including past treatments and investigations. Do not refer patients that are currently being investigated for RED FLAG pain signs and symptoms as these will be returned until appropriate investigations are completed.

NOTE: Our physicians do not prescribe opioids but can provide recommendations on pharmacological pain management if required.
